About

Get Well Family Health Team is a not-for-profit, community-focused primary care practice serving North York residents — particularly those in the M2N and M3H postal code areas — who currently do not have a family doctor. The team is collaborative and interdisciplinary: family physicians and nurse practitioners work alongside registered dietitians, social workers, physiotherapists, a psychotherapist, an onsite pharmacist, and a chiropractor to deliver wholistic, patient-centred care that goes well beyond a standard GP visit. The team office is at 4430 Bathurst Street, with the Access Clinic at 649 Sheppard Avenue West. Registered patients benefit from a wide range of in-house programs: diabetes care, anxiety support, lower back pain management, women's health screenings, traditional healing services, medication review, and flu shot clinics. The multilingual team currently serves patients in English, Mandarin, Cantonese, and Vietnamese, with plans to expand as the practice grows. Same-day and after-hours appointment access is available for registered patients. Residents of North York with OHIP and no current family doctor are encouraged to register online.
