Black Creek Community Health Centre - Sheridan Mall
About
Black Creek Community Health Centre has been a cornerstone of health and wellness in northwest Toronto for decades, providing integrated, barrier-free services to the diverse communities around Jane Street and Finch Avenue. Operating from two locations — Sheridan Mall and Yorkgate Mall — the centre is strategically situated within the daily lives of residents, making healthcare genuinely accessible to those who may otherwise face financial, linguistic, or systemic barriers. As a non-profit, community-governed organization, BCCHC ensures that services remain free, culturally responsive, and rooted in the actual needs of the people it serves. The centre offers a comprehensive range of health services including primary care, sexual health, diabetes education, lung health programs, physiotherapy, mental health counselling, nutritional support, foot care, pregnancy and breastfeeding support, and kinesiology. Beyond clinical care, BCCHC delivers vibrant community programming — from children's early years programs and youth supports to seniors' groups and harm reduction services. The health centre's team speaks many of the community's languages and is committed to fostering an environment where every client feels safe, respected, and celebrated.
