Black Creek Community Health Centre - Yorkgate
About
Located inside Yorkgate Mall at 1 York Gate Boulevard in North York, Black Creek Community Health Centre's Yorkgate site brings comprehensive community health services directly into one of Toronto's most diverse and high-needs neighbourhoods. The Yorkgate location mirrors the full breadth of services available across BCCHC's network, including primary care, sexual health, diabetes education, foot care, mental health counselling, nutrition support, physiotherapy, and lung health programming. By embedding care inside an accessible mall location, BCCHC dramatically reduces the practical barriers that often prevent people from seeking help. Black Creek CHC operates as a non-profit community health model, meaning that care is delivered by a multidisciplinary team that addresses not just clinical needs, but the social determinants of health that affect life in northwest Toronto. Programs support newcomers navigating the Canadian health system, seniors managing chronic illness, youth facing mental health challenges, and families working toward healthier lifestyles. The Health Promotion and Addictions Team provides both one-on-one and group-based supports. For residents of Jane and Finch, Humber Summit, and the surrounding North York communities, the Yorkgate site is a vital neighbourhood resource. Extended hours Monday through Thursday from 8:00 AM to 7:30 PM, and Saturday availability from 9:00 AM to 5:00 PM, make care accessible even for those with demanding work and family schedules.
