Don Mills Family Health Team is a community-focused family medicine practice located at 20 Wynford Drive, Suite 200, in the Don Mills neighbourhood of North York, affiliated with Sunnybrook Health Sciences Centre. The practice is actively welcoming new patients to the family practices of Dr. Jimmy Ha and Dr. Anaeis Nazarians, with additional availability through Nurse Practitioner Matthew Somerset and Dr. Jesse Weinberg.
