Flemingdon Health Centre
About
Flemingdon Health Centre (FHC) is a community-based, non-profit organization that has been providing free, barrier-free health and social services to residents of Flemingdon Park, Thorncliffe Park, and surrounding north-east Toronto neighbourhoods for 50 years. With two main locations — at 10 Gateway Boulevard and 5 Fairview Mall Drive — FHC serves one of the most ethnically diverse and under-resourced communities in the city, offering care without regard to OHIP status, immigration status, or ability to pay. FHC's multidisciplinary services include primary care by physicians and nurse practitioners, integrated mental health services, allied health (physiotherapy, dietetics, social work), diabetes prevention and management, health promotion programs, and the Health Access Thorncliffe Park program which delivers care directly in the Thorncliffe Park community hub. The centre also operates the Chronic Disease Management Hub, youth wellness supports, and a range of community programs. Whether patients need a family doctor, mental health support, or help navigating the health system, FHC's collaborative team is there to walk alongside them.
