Health Access Thorncliffe Park
About
Health Access Thorncliffe Park is a community-based primary health care clinic serving the residents of the M4H postal code, a densely populated, culturally diverse area in East York. Operated in partnership with Flemingdon Health Centre, the clinic provides free, accessible health services to residents regardless of insurance status or health card. No referral is required to access care, making it a critical resource for newcomers, low-income residents, and those who have fallen through the cracks of the traditional health care system. The clinic's services are delivered by a multidisciplinary team including physicians, nurse practitioners, social workers, nutritionists, and health educators. Services include primary care, social work support, nutrition counselling, family support programs, health education and promotion, foot care, and a dedicated pediatric clinic. The team takes a holistic approach that addresses both the medical and social determinants of health. Conveniently located within the Thorncliffe Park community, the clinic is open Monday through Friday with evening hours on Tuesdays and Thursdays, and select Saturday hours twice a month. Its accessible, no-barrier model of care makes it an invaluable community asset for one of Toronto's most underserved and diverse neighbourhoods.
