The North York Community Care Clinic (NYCCC) can connect you with a nurse practitioner on a temporary basis to ensure you receive necessary care while you wait to be connected to a permanent primary care provider (family doctor or nurse practitioner). Eligibility criteria To access the North York Community Care Clinic: You cannot have an existing family doctor or nurse practitioner. You must live close to the clinic(s) below and be able to attend in-person.
