About

Scarborough Centre for Healthy Communities (SCHC) is one of the largest and most comprehensive community health organizations in the eastern GTA, serving residents across Scarborough with a deeply integrated range of health, wellness, and social support services. Founded on the belief that good health extends far beyond a clinic visit, SCHC addresses the full spectrum of factors — social, environmental, and medical — that shape the wellbeing of Scarborough's remarkably diverse population. SCHC offers primary care through a dedicated team of physicians and nurse practitioners, a robust diabetes education program, mental health services, hospice palliative care coordination, seniors day programming, a home support program, a food bank, and much more. The organization also operates a mobile health unit and partners with community organizations to reach individuals who may not otherwise access care. With its Tamil, South Asian, Southeast Asian, and Caribbean programming, SCHC is a vital lifeline for Scarborough's multicultural communities.
