Situated on Weston Road in the Mount Dennis neighbourhood, Crosstown Family Health Team brings together a multidisciplinary group of physicians and allied health professionals committed to delivering comprehensive, team-based primary care. The Family Health Team model means patients aren't just seeing one doctor — they're supported by a coordinated network of professionals including nurses, dietitians, social workers, and mental health counsellors, all working toward a unified care plan. Crosstown FHT is affiliated with Toronto Metropolitan University and actively accepts new patients through phone registration or walk-in during designated doctor hours. The clinic runs extended weekday hours from 8 AM to 8 PM Monday through Thursday, ensuring working families and busy residents can access care outside of traditional office hours. After-hours advisory support is also available. Located at the corner of Weston Road and Jane Street, the clinic is easily reached by TTC and serves one of Toronto's most diverse west-end communities.
