Davenport-Perth Neighbourhood and Community Health Centre
About
In the heart of Toronto's west end, the Davenport-Perth Neighbourhood and Community Health Centre has been a foundational resource for the community since 1985. DPNCHC operates as a multi-service agency that provides integrated, accessible health and community services to individuals and families who face social and economic barriers — particularly newcomers, uninsured residents, racialized groups, and other vulnerable populations who often encounter obstacles in the mainstream healthcare system. The centre's Health Services team can be reached directly at 416-658-6812 and offers primary medical care alongside a wide range of programming for adults, children, youth, and seniors. Health promotion, chronic disease management, early childhood programs through the EarlyON Child & Family Centre, and community development initiatives all operate under one roof. DPNCHC is accredited by the Canadian Centre for Accreditation and governed by a community-elected Board of Directors — a structure that ensures the centre remains directly accountable to the people it serves.
