Regent Park Community Health Centre - Diabetes Education Program
About
The Regent Park Community Health Centre Diabetes Education Program, located at 563 Dundas Street East (Suite 205–206), provides structured diabetes education and self-management support to individuals living with or at risk of diabetes in the Regent Park and surrounding communities. The program is delivered by a multidisciplinary team including registered dietitians, nurses, and other certified diabetes educators who work together to help clients understand and manage their condition. The program takes a patient-centred, culturally sensitive approach, recognizing that effective diabetes care must be grounded in a person's language, culture, and daily life. Group education sessions, individual consultations, and ongoing support are available. The program operates Monday, Wednesday, Thursday, and Friday from 10:00 AM to 4:00 PM, and Tuesday from 12:00 PM to 4:00 PM. Referrals from a physician or nurse practitioner are typically required.
