Village Family Health Team is an interdisciplinary primary care practice at 171 East Liberty Street in Toronto's Liberty Village neighbourhood, bringing together family physicians, nurse practitioners, nurses, social workers, and dietitians under one roof. Currently accepting new patients with priority given to residents in the M6K and M6J postal codes, the team combines the clinical depth of a full family health team with an accessible west-end location that's easy to reach by transit or on foot. Programs and services span the full range of primary care and preventative health: cancer screening, prenatal care, well baby visits, metabolic health management, mental health support, substance use services, nutrition counselling, and flu vaccination clinics. Hours run Monday through Thursday until 8:00 PM and Saturdays until 3:00 PM — giving patients genuine flexibility for both routine and urgent needs.
